OPG & CBCT Scan Referrals

We take referrals for OPG and CBCT Scans

  • Please use the attached form to refer your patients to us for an OPG or CBCT scan.
  • Please note that referring dentist must be Level 1 qualified to refer for CBCT and must have a Level 2 clinician available to report on the scan.

We accept referrals for patients needing OPG or CBCT scans. You can upload relevant information to our Referral Form.
