We take referrals for OPG and CBCT Scans

Please use the attached form to refer your patients to us for an OPG or CBCT scan.
Please note that referring dentist must be Level 1 qualified to refer for CBCT and must have a Level 2 clinician available to report on the scan.

We accept referrals for patients needing OPG or CBCT scans. You can upload information and any supporting x-rays, images to our Referral Form
